Nurse Liaisons

The Complete Care Group utilizes qualified Nurse Liaisons to help connect clients with needed healthcare services. Our liaison is available to meet with clients by appointment and holds regularly scheduled office hours at each Complete Care Group community, acting as a central point of contact for clients, family members, staff, and our preferred healthcare providers. Upon meeting with each new client, our liaison is able to make recommendations and referrals to needed services based on information obtained during the initial needs assessment.


Kristen Bragelman, RN

Kristen was born and raised in a small town near Marshfield, Wisconsin. She grew up on a dairy farm, where her parents still live and continue to do farm work. Kristen always wanted to be a nurse and have a career working with the geriatric population. She received her Associate’s Degree in Nursing from Anoka-Ramsey Community College and her Bachelor’s Degree in Human Services and Gerontology from Metropolitan State University.

With a passion for older adults—particularly those with memory loss—Kristen has worked in senior settings since 2000. She has had roles that include caregiving, resident activities, marketing and nursing. She has also worked in a hospital setting as a staffing coordinator, and as an RN on an orthopedic /spine unit. She finds it so rewarding to bring joy, even just for a moment, to the residents and patients she serves.

When not in the office, Kristen enjoys spending time with her husband and two children—and their crazy Goldendoodle!


Sara Lynaugh, RN

Sara grew up in Spencer, a small town in central Wisconsin. She attended UW-Eau Claire where she received a degree in Business Management, with an emphasis in Human Resources. After working in the insurance industry for a few years she realized that the medical field—with face-to-face connections—was her calling. And since her mom and brother were both nurses—nursing seemed to be in her blood. So in 2016 she received her Associates of Applied Science in nursing from Rasmussen College. 

Sara’s favorite parts of her current work are the interactions with her clients and co-workers. They are the reason she gets up in the morning with a smile on her face. She loves her job—and it shows! Some of her most memorable moments have been the special work she has done with hospice clients. She is proud of Legacy’s work coordinating care to help families as much as possible during this difficult time.

When not in the office, Sara is a triathlete. Long runs, bike rides and time spent swimming keep her busy. Other passions include her family, friends, music—and her kitty!



Karissa Borchert, MSW, LGSW

Karissa is originally from a small town in southwestern Minnesota and currently resides in Plymouth, MN. She graduated from Minnesota State University, Mankato with a Bachelor of Science in Psychology and completed her Masters of Social Work (MSW) from the University of Minnesota.

Karissa has worked in a variety of social service and medical settings most frequently with adults and elders with mental illnesses and neuromuscular diseases. She is a Qualified Developmental Disability Professional (QDDP), and has received her LGSW.

Karissa understands that each client has unique needs and works to ensure they receive the best care and services possible. Karissa enjoys working with a client's entire team, including family members and referral sources to make a transition as smooth as possible.

When not in the office, Karissa keeps busy with her little boy, training for and running marathons, golfing, traveling, and going to concerts.
